Medical Records Department


The Health Information Department at Southeast AZ Medical Center, is open Monday through Friday from 8:00 am to 12:00 and 12:30 to 4:30. We are located at the back of the hospital down the hall from the radiology department. Copies of your medical records can be obtained through us during our office hours. We have a Medical release form that we will ask you to fill out when obtaining those records. We can also fax directly to a Dr's office per your request for appointments. When you are picking up records please bring your drivers license or a federally accepted form of ID for identification.
